29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway was not kept free from protruding nails, splinters, holes, or loose boards:      Numerous pot holes were observed throughout the terminal where extensive fork truck and pedestrian traffic occurred.  The most egregious pot holes existed at the following locations:      a/  Adjacent to Dock #9, one pot hole that measured approximately 10"W X 1"D X 14"W.      b/  Adjacent to Dock #21, one pot hole that measured approximately 10"W X 2"D X 12"W.      c/  Adjacent to Dock #66, one pot hole that measured approximately 60'L X 2"D X 4"W.      d/  Adjacent to Dock #72, one pot hole that measured approximately 15'L X 2"D X 12"W.      e/  Adjacent to Dock #91, one pot hole that measured approximately 22"L X 2"D X 12"W.      f/  Adjacent to Dock #73, one pot hole that measured approximately 150"L X 2"D X 12"W.      g/  Adjacent to Dock #33, two pot holes that measured approximately 4'L X 2"D X 12"W.

29 CFR 1910.24(f):  Fixed stairs did not have uniform rise height and tread width throughout the flight of stairs:  a/  At the employee front entrance to the Terminal, the tread belonging to a flight of concrete, fixed stairs was not uniform.  The fixed stairs consisted of six risers and treads.  The third tread measured approximately 12 inches on the right from the base.  The same tread measured approximately three inches on the left.

The energy sources include gravity and residual electrical energy.

29 CFR 1910.252(a)(2)(vii):  Where relocation of combustible material was impracticable, cutting or welding was permitted when combustibles were not protected with flameproof covers or otherwise shielded with metal, asbestos guards, or curtains:    a/  Inside of the Maintenance Shop, oxy-fuel cutting and MIG welding occurred within 35 feet from a 55-gallon drum partially filled with motor oil.  No suitable covers or the equivalent were used.
