29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: 4101 Washington Ave., Newport News, VA 23607: On or about November 23, 2015, and times prior thereto, the employer did not enter the following injuries or illnesses on the OSHA 300 Log or equivalent for the calendar year 2015: a) November 23, 2015, on ship, injured knee in a fall that resulted in job transfer or restriction was not recorded. b) October 20, 2015, in shop, injured ankle when struck by an object that resulted in job transfer or restriction was not recorded. c) October 19, 2015, Mechanic, injured elbow from forceful gripping that resulted in job transfer or restriction did not correctly record the job transfer or restriction. d) September 22, 2015, Shipfitter, injured shoulder that resulted in job transfer or restriction was not recorded.
