29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   (a) Jobsite:  On or about October 6, 2015, two employees were working from a ladder jack scaffold and they were not protected from falling to the ground.  The fall hazard to the employees was about 13 feet to the ground.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a)  Jobsite:  On or about October 6, 2015, at least three employee were working on a steep roof and they were not protected from falling to the ground.  The ground to eave height is about 14 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:   (a) Jobsite:  On or about October 6, 2015, at least three employees who were working on the roof and were exposed to falling from the roof need to be retrained so that they can identify fall hazards and can take the necessary steps to minimize these fall hazards.
