29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite: On or about August 29, 2016, three employees were working from a residential roof and they were not protected from falling. The fall hazard to the employees ranged from about 20 feet to about 25 feet.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards: (a) Jobsite: On or about August 29, 2016, two employees who were working on the roof were exposed to falling from the roof need to be trained so that they can recognize fall hazards and can take the necessary steps to minimize these fall hazards.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill: (a) Jobsite: On or about August 29, 2016, at least one employee who was working on the roof and was exposed to falling from the roof needs to be retrained so that he can identify fall hazards and can take the necessary steps to minimize these fall hazards.
