29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a.) Osram Sylvania, Inc.,835 Washington Rd., Saint Marys, Pa, Production areas, On or about October 8th, 2015, and times prior thereto: Employees performed servicing and maintenance work on machines/equipment that included, but was not limited to, HP-1000 Blister former machine (Serial # 61217398) and Unit #12 machine. A periodic inspection had not been conducted with the authorized employees that perform servicing/maintenance work on the machines/equipment. The last periodic inspection had been conducted greater than 2 years ago.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.
