Employees in the facility were exposed to hazards associated with evacuation in the event of a fire or an anhydrous ammonia leak. The employees were exposed in that sufficient egress was not given: a) At the Front Press New Bakery, the exit access was found to be approximately 18.5 inches wide. b) At the Crust Table, the exit access was found to be approximately 18 inches and 20 inches wide.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place: An employee operating a RAYMOND forklift (#626 Model 4450) in the production and warehouse areas, as well as employees in near proximity, were exposed to struck-by hazards. The employees were exposed in that the control levers which move the forks were missing the markings to indicate which direction the operator should move the levers to avoid striking someone or something.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 09/30/15, an employee engaged in production activities, including the unjamming of pans, at Blast Furnace #5 was exposed to amputation hazards in that the out feed conveyor was not guarded b) An employee engaged in set-up activates at Blast #4 Freezer out feed conveyor was exposed to amputations in that machine guarding was not provided. The Schwan's Global Supply Chain Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1) which was contained in OSHA inspection number 941314, citation number 1, item number 18 and was affirmed as a final order on 03/27/14, with respect to a workplace located at One Lemon Lane, Atlanta, GA 30307.
