Carpentry shop, tool room; on or about 07 October 2015 and times prior; a 25 foot yellow extension cord was missing the ground pin. This condition exposed employees to an electrical shock hazard.

Carpentry shop, tool room; on or about 07 October 2015 and times prior; a yellow temporary lighting string had a damaged lamp socket. This condition exposed employees to an electrical shock hazard.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.  Building 615, east side; on or about 07 October 2015 and times prior thereto; the work rest on a Black and Decker Bench Grinder, serial number 896901; was not properly adjusted to a maximum of one-eighth of an inch from the abrasive wheel to the work rest. The gap between the abrasive wheel and work rest measured approximately one-quarter of an inch. This condition exposed employees to a caught in hazard.  The Directorate of Public Works was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.215(a)(4), which was contained in OSHA inspection number 711238, citation number 2, item number 1 and was affirmed as a final order on 17 December 2012, with respect to a workplace located at the Build 615 Roads and Grounds Shop.

Plumbing shop; on or about 07 October 2015 and times prior; an energized four plug junction box was wire to an extension cord. This condition exposed employees to an electrical shock and burn hazard.     The Directorate of Public Works was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 711238, citation number 2, item number 2 and was affirmed as a final order on 17 December 2012, with respect to a workplace located at the Plumbing Shop.

Carpentry shop; on or about 07 October 2015 and times prior thereto; A knockout plug was missing from a 240 volt energized electrical junction box. Junction box supplies power to operate a table saw. This condition exposed employees to electrical shock and electrical burn hazard.  The Directorate of Public Works was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 711238, citation number 2, item number 3 and was affirmed as a final order on 17 December 2012, with respect to a workplace located at the Plumbing Shop.
