29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  Auto shop; on or about 06 October 2015 times prior thereto, the emergency exit door was locked during business hours. A keyed Master lock was attached via a hasp to the exit door. This condition exposed employees to smoke and fume inhalation, entrapment and fire hazards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.  Auto shop; on or about 06 October 2015 and times prior thereto; the work rest on a bench grinder, make and model not legible was not properly adjusted to a maximum of one-eighth of an inch from the abrasive wheel. Two instances were noted:      a. On the left hand side; the gap between the abrasive wheel and work rest measured approximately 7/16 of an inch.      b. On the right hand side; the gap between the abrasive wheel and work rest measured approximately 1/2 of an inch.  This condition exposed employees to a caught in hazard.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting.  Bowling alley, Pin set area; on or about 06 October 2015 and times prior thereto, the employer did not ensure that horizontal shafting was guarded. The chain tensioner shafts on eight of the pinsetter machines were not guarded, exposing employees to rotating parts. This condition exposed employees to a caught in hazard.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.  Outdoor recreation, on or about 06 October 2015 and times prior thereto; the employer did not ensure that the electrical panels were readily accessible. Two instances were noted:       a. Behind the main service counter area; a copying machine was stored in front of the electrical panel labeled 01787 impeding immediate access.          b. In the customer service area, located in front of the main counter; an electrical panel had a desk and picture board stored in front of the panel impeding immediate access.  This condition exposed employees to an electrical shock hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.   Auto shop and bowling alley; on or about 06 October 2015 and times prior; four instances were noted where energized electrical openings were not effectively closed:       a. Auto shop bay 6, circuit labeled 18 - an energized electrical outlet had two knockout plugs missing on the right hand side.       b. Auto shop tool room south wall - an energized electrical outlet had a knockout plug missing on the right hand side.        c. Bowling alley pinsetter area, adjacent to humidifier drain pipe - an energized electrical box had a knockout plug missing from the top of the box.   This condition exposed employees to electrical shock and electrical burn hazard.    The U.S. Army Directorate of Family and Morale, Welfare and Recreation was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 711518, citation number 2, item number 1 and was affirmed as a final order on 12 December 2012, with respect to a workplace located at the Auto shop and Bowling alley.

Auto shop and Outdoor Recreation Shops; on or about 06 October 2015 and times prior thereto; two instances were noted where strain relief was not provided:       a. Auto shop, bay 6; strain relief was not provided to a blue 25 foot extension cord.        b. Outdoor recreation work bench area; strain relief was not provided on the cord to power the work bench outlets.  This condition exposed employees to electrical shock and burn hazards.   The U.S. Army Directorate of Family and Morale, Welfare and Recreation was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 712100, citation number 1, item number 2 and was affirmed as a final order on 12 December 2012, with respect to a workplace located at the Auto shop and Outdoor recreation.
