29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.    Building 15: On or about November 3, 2015 and at times prior thereto, the mezzanine was not marked with the load rating and was used for storage of maintenance repair equipment and mechanical parts, exposing employees underneath to potential falling hazards.

29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.    Building 200 - Penthouse: On or about October 7, 2015 and at times prior thereto, puddles of water had collected on the floor and under the steel threshold ramp used by employees to enter the rooftop and maintenance areas creating a slipping hazard.     The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(2) which was contained in OSHA inspection number 1074778, notice number 2, item number 1 and was affirmed as a final order on August 18, 2015, with respect to a workplace located at 2615 E. Clinton Avenue, Fresno, CA 93703. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    Building 203 Warehouse: On or about October 6, 2015 and at times prior thereto, a surge protector with and eight foot extension was used as fixed wiring for General Electric microwave and Kenmore refrigerator and toaster in place of fixed wiring, exposing employees to electrical hazards when using the appliances.      Building 203/1A Breakroom:  On or about November 19, 2015 and at times prior thereto, a surge protector with portable extension cord was used as fixed wiring for microwave, toaster and coffee makers in place of fixed wiring, exposing employees to electrical hazards when using the appliances.      Building 20 Groundskeeping: A portable relocatable power tap (yellow) was used as temporary wiring to power to a microwave  - Mfg. General Electric  II S/N MT9474742 and Kenmore refrigerator  exposing employees using equipment to electrical hazards.    The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.303(b)(2) which was contained in OSHA inspection number 1074778, notice number 2, item number 2 and   was affirmed as a final order on August 18, 2015, with respect to a workplace located at 2615 E. 29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    Building 15 - Maintenance:  On or about November 3, 2015 and at times prior thereto, access to the electrical load center and transformer shutoff controls were blocked by storage of steel racks, trash bins, compressed gasses, and tools.  The control for the transformer provided temporary power to the outside trailer was blocked and prevented direct access to shut down equipment for operators and maintenance employees- Manufacturer(s): Cutler Hammer and International Transformer Corporation.    Building 200 - Penthouse:  On or about October 7, 2015 and at times prior thereto, access to the electrical panels and breaker boxes near (AHU-6Surgery and at pumps 200-CWP-1-2 ) was blocked by items approximately eight feet high including, but not limited to, cardboard box, chair and air filters which were stored in front and next to the water pumps at 200-CWP-1-2. The electrical disconnect were not readily accessible to employees in the work area.     Building 208 - Community Living Center:  On or about November 23, 2015 and at times prior thereto, access to the electrical panels and breaker boxes were blocked with material being stored including ladders, dollies, buckets with tools in Electrical Room -Critical Branch #208-GC-ELP2 208Y 3P/4W 225A;#208-GC-ELP3 208Y 3P/4W 225A.    The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.303(g)(1) which was contained in OSHA inspection number 110227, notice number 1, item number 06 and   was affirmed as a final order on June 12, 2013, with respect to a workplace located at 10535 Hospital Way, Mather, CA 95655.    The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.303(g)(1) which was contained in OSHA inspection number 678158, notice number 03, item number 03 and was affirmed as a final order on May 17, 2013 with respect to a workplace located at 3801 Miranda Avenue, Palo Alto, CA 94304. 29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:     VAMC San Francisco: On October 7, 2015, and at times prior thereto, electrical equipment had unused openings that could allow unintentional contact with live inner conductors exposing employees to electrical hazards in the following locations:    A. Building 203, Loading Dock: Electrical box had a missing knockout, it provides hallway power near the emergency medical oxygen connection exposing warehouse, delivery and food service and maintenance workers to electrical hazards.    B. Building 201 3rd Level Penthouse: Electrical safety switch had an opening and exposed wiring one inch by one and a half on the faceplate which created an electrical hazard to maintenance workers servicing areas # 203-EF-26 LRB CKT4.    C. Building 201 3rd Level Penthouse: Electrical  safety switch had a missing knockout at the electrical service disconnect isolation  for the Air Purification units which created a potential hazard and exposed maintenance workers servicing areas # 203-EF-2 MCC-AE 38, 40,42.    D. Building 200: Rooftop Area: Electrical safety switch had a missing knockout at the electrical service disconnect isolation creating a potential hazard and exposed maintenance workers servicing areas # 200-CU 6 BLAB/RA 25,27,29.                          E. Building 200: Rooftop Area: Electrical safety switch had a missing knockout at the electrical service disconnect serving areas  # 200-AHU-8 200-MCCC-E # 68,70,72 Motor #1 exposing maintenance workers to electrical hazards.    F. Building 30/Pump House: Electrical box had seven missing knockouts for panel # EV/EVI for pump #2,  exposing maintenance workers to electrical hazards.    The Department of Veterans was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 110227, notice 1, item number 7,  and was affirmed as a final order on June 12, 2012, with respect to a workplace located at 10535 Hospital Way, Mather, CA 95655. 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    Building 200-Rooftop- 3rd Level Penthouse: On or about October 7, 2015 and at times prior thereto, a 50 foot extension cord regularly used was found with two sections having outer insulation damage exposing inner wiring and created a hazard. These cords are used weekly for temporary power for air handler pumps or lighting during maintenance and exposed employees to electrical hazards.    Building 203/1A-Inpatient Care: On or about November 19, 2015 and at times prior thereto, a warming cabinet linen exchange with a cord and plug connected had damaged insulation at the plug which exposed employees handling the equipment to electrical hazards - Manufacturer Enthermics Medical Systems.    The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334(a)(2)(i), which was contained in OSHA inspection number 1074778, notice number 3, item number 1, and was affirmed as a final order on August 18, 2015, with respect to a workplace located at 2615 E. 29 CFR 1910.141(a)(5): A continuing and effective extermination program was not instituted where rodents, insects, or other vermin were detected:    Building 20: On or about October 8, 2015, and at times prior thereto, mice activity showing mice droppings in the cabinets that employer failed to maintain an effective program and prevent the entrance or harborage of rodents, insects, or other vermin in the lockers used by the maintenance and landscaping employees who used the lockers daily and continued for over a month.
