OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using an automotive lift:    (a)  On or about October 13, 2015, and at times prior the employer did not ensure that employees         were safe from crushing and struck-by hazards in that the following lifts were noted with deficiencies:        (1)  Challenger Lift, model E10, serial #150800000128630, did not have the locking mechanism locked             into place while the lift was raised and in use.        (2)  Hydraulic in-ground automotive lift, located in bay 2, does not have a locking mechanism to keep the              lift in place while in use.          (3)  Challenger Lift, model E10, serial #150800000128634, did not have the locking mechanism locked into            place while the lift was raised and in use.                   (4)  Auto lifts were not periodically inspected by a qualified inspector at least annually to ensure that             they were properly maintained and functioning.            (5)  The employer did not document that all lift users were trained to use the lifts.
