29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a) Lower Level - An electrician was observed installing a new temporary light circuit in an unguarded service panel and exposed to live electrical parts, 120 volt, 200 amp service, on or about 10/13/2015.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:    a) Exterior, West Wall - Laborers were observed on a tubular welded frame scaffold work platform, 7 ft. wide and  32 ft. high,  handing material to bricklayers on the second story balcony and the scaffold was not secured, on or about 10/13/2015.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:    a) Exterior, West Wall - Laborers were observed standing on a scaffold work platform 14 feet above the ground and 5 cross braces were missing on the ground level sections, exposing laborers to a collapse hazard, on or about 10/13/2015.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1052(b)(2): Foot traffic was not prohibited on skeleton metal stairs where permanent treads and/or landings were to be installed at a later date and the stairs were not fitted with secured temporary treads and/or landings long enough to cover the entire tread or landing area:    a) Apartment Stairway - Masons and laborers were using a metal stairway with a three quarter inch pan and exposed to a trip and fall hazard, on or about 10/13/2015.
