29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  (a)   28 SOUTH VINTAGE ROAD, OUTSIDE WORK AREA - Employee(s) used a 100 foot extension cord to supply electricity while using a Hougen HMD904 Magnetic Drill, and the extension cord had sections of the outer insulation that were missing and damaged which exposed the electrical conductors underneath, thereby exposing the employee(s) to electrical shock injuries, on or about October 13, 2015.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
