29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment shall be consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience. (a) Corden Pharma Colorado, Inc. at 2075 North 55th Street, Boulder, CO: On and preceding 10/08/15, the employer did not follow a maintenance schedule advised by the manufacturer for the Goulds centrifugal pump, labeled PP-PS-148 on the P&ID. Maintenance inspections were not conducted in accordance with manufacturers instructions regarding components that were in contact with corrosive liquids. This condition resulted in a release of flammable liquids and/or highly hazardous chemicals into the atmosphere.

29 CFR 1910.119(j)(6)(iii): The employer did not assure that maintenance materials, spare parts, and equipment were suitable for the process application for which they will be used: (a) Corden Pharma Colorado, Inc., at 2075 North 55th Street, Boulder, CO: On and preceding 10/08/15, the employer used an impeller sleeve manufactured by Peerless to replace an impeller sleeve on a Goulds centrifugal pump, labeled PP-PS-148 on the P&ID. This condition resulted in a release of flammable liquids and/or highly hazardous chemicals into the atmosphere.
