29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a.) Fahey Exteriors, LLC., Metropolitan Baptist Church: On or about 10/13/15, employees applying waterproofing to the roof were not protected from falling 19.5' to the concrete ground below. Fahey Exteriors, LLC.,, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 903512, Citation 01, Item number 005 and was affirmed as a final order on 08/30/13, with respect to a workplace located at 932 Central Ave., Charleston, WV 25302. ABATEMENT DOCUMENTATION IS REQUIRED.
