29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about October 15, 2015 employees were not required to lockout the Cincinnati Press Brake while performing setup work and changing dies.    b) On or about October 15, 2015 employees were not required to lockout the HTC Press Brake while performing setup work and changing dies.     c) On or about October 15, 2015 employees were not required to lockout the Standard Press Brake while performing setup work and changing dies.     d) On or about October 15, 2015 employees were not required to lockout the AccurPress Press Brake while performing setup work and changing dies.     e) On or about September 29, 2015 employees were not required to lockout the Universal Punch Press while performing setup work and changing dies.     f) On or about October 15, 2015 employees were not required to lockout the Heim Punch Press while performing setup work and changing dies.     g) On or about October 15, 2015 employees were not required to lockout the Rousselle Punch Press while performing setup work and changing dies.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) On or about October 15, 2015 the employer did not train authorized employees performing die changes and setup activities on press brakes and pinch presses in the proper application and removal of energy control procedures nor in the recognition of hazardous energy sources present.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) On or about October 15, 2015 employees who performed press setup activities and die changes on the four press brakes and punch presses, were not required to:  (1) knowing the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy;  (2) turning off and shutting down the machine;  (3) physically locating and operating energy isolating devices in such a manner as to isolate the machine or equipment from the energy source;  (4) affixing lockout or tagout devices to each energy isolating device;  (5) prior to starting work on machines that have been locked out or tagged out, verifying that isolation and de-energization of the machine or equipment have been accomplished.  Employees required to rely upon equipment emergency stop circuitry were exposed to crushing hazards from unexpected and/or unintentional downward movement of the top ram on the press brakes and punch presses.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) On or about October 15, 2015 the point of operation on the Cincinnati 175 ton Press Brake was not guarded to prevent operator exposure during the hazardous downward ram stroke.     b) On or about October 15, 2015 the point of operation on the HTC Press Brake was not guarded to prevent operator exposure during the hazardous downward ram stroke.     c) On or about October 15, 2015 the point of operation on the Standard Press Brake was not guarded to prevent operator exposure during the hazardous downward ram stroke.     d) On or about October 15, 2015 the point of operation on the AccurPress Press Brake was not guarded to prevent operator exposure during the hazardous downward ram stroke.     e) On or about September 29, 2015 the point of operation on the Universal Punch Press was not guarded to prevent the operator and/or the operator assistant from having any portion of their body exposed to the hazardous downward ram stroke.

29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to insure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment:    a) On or about October 29, 2015 the employer had not established and implemented a periodic inspection program for the Rousselle punch press to ensure that all parts and safeguards were in a safe operating condition and adjusted properly.
