29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Automotive Shop, where the FMC brake lathe was not equipped with a guard.  The U.S. Exchange Autoport was previously cited for a violation of this standard which was contained in OSHA inspection no. 314898743, citation no. 2, item no. 1, issued on March 4, 2011 with respect to a workplace at Silverdale, WA.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:    a) Facility, where equipment not used as listed or labeled included:    1. Automotive Shop, where a relocatable power tap equipped with a damaged outer casing was used to supply power to the work bench light fixture and portable tool battery chargers.  2. Inventory Clerk Office, where a relocatable power tap was used to supply power to appliances, including the Titan portable floor heater.  (Item Corrected)   The U.S. Exchange Autoport was previously cited for a violation of this standard which was contained in OSHA inspection no. 314898743, citation no. 2, item no. 2, issued on March 4, 2011 with respect to a workplace at Silverdale, WA.
