A lockout/tagout procedure was not developed for the 4C line filler. As an employee was attempting to remove solidified product from the filler, the machine completed a cycle and amputated the tip of the employee's finger, on or about October 9, 2015.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
