29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer did not develop, document, and utilize procedures for the control of potentially hazardous energy when employees were engaged in activities covered by this section. A lockout/tagout procedure was not developed for the 4C line filler. As an employee was attempting to remove solidified product from the filler, the machine completed a cycle and amputated the tip of the employee's finger, on or about October 9, 2015. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
