Location: c/o Broadway and Bloomfield Ave., Newark, NJ 07114  a) Employees used an extension cord that was missing the ground pin to power a Makita grinder.  The circuit lacked a continuous path to ground; on or about 10/15/15.

Employees were exposed to a potential fall hazard of up to approximately 12 ft. to the floor below; on or about 10/15/15.

Location: c/o Broadway and Bloomfield Ave., Newark, NJ  a) Employees were not protected from falling while working from a mason scaffold that did not have a guardrail system and they were not wearing a personal fall arrest system.  The employees were exposed to a potential fall of up to approximately 12 ft. to the floor below; on or about 10/15/15.
