Condition occurred on/or about October 13, 2015.

29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of American National Standards Institute, B7.1-1970, Safety Code for the Use, Care and Protection of Abrasive Wheels, and paragraph (d) of this section: a) VA New Jersey Health Care System - Lyons Campus - 151 Knollcroft Road, Lyons, NJ 07939 - Circle 2: Employees were permitted to perform concrete grinding operations utilizing a Makita 4-1/2" SJS" High-Power Angle Grinder (Model 9564CV), without the use of safety guards. Condition occurred on/or about October 13, 2015.
