29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. On or about October 15, 2015 at the above addressed job site, employees were exposed to caught by hazards while working with an HPM 45-87 hydraulic press break without point of operation guarding and/or alternate measures that provides effective protection.      b. On or about October 15, 2015 at the above addressed job site, employees were exposed to cuts and lacerations from flying chips while working with a SHARP-164L lathe without guarding.    c. On or about October 15, 2015 at the above addressed job site, employees were exposed to cuts and lacerations from flying chips while working with Bridgeport drill press TCT 163, Bridgeport drill press TCT 137, Lagune Republic FTU-2 drill press and Jones-Shipman 310T drill press without guarding.    d. On or about October 15, 2015 at the above addressed job site, employee was exposed to caught in hazard while working with HE&M H101-A1 horizontal band saw that lacked guarding for the unused portion of the cutting blade.
