29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  In PK-3 an operator on the Hayssen Packaging Machine 4043 reached into the machine in order to jam a piece of cardboard in between the date set-up rollers which were malfunctioning.  The employee was exposed to amputation hazards while performing this servicing task.  The employee was not required to:   (1) Know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy; (2) Turn off and shut down the machine; (3) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source; (4) Affix lockout or tagout devices to each energy isolating device; (5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.  The employee did not isolate the machines from all energy sources.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(m)(2): Employee(s) stood under or passed under the elevated portion of a powered industrial truck(s):  On or about September 21, 2015 and continuing to date, employees on the powdered sugar line in PK-3 are exposed to a struck-by hazard when they pass under the elevated portion of a powered industrial truck, with their arms, in order to open a tote suspended over a hopper.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:   On or about September 21, 2015, an employee on the powdered sugar line in PK-3 was exposed to a struck-by hazard when he passed under the elevated portion of a powered industrial truck, with his arm, in order to open a tote suspended over a hopper.  The load was not stable as the employee was handling it.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
