29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition:    a) Press #4 Area: This violation was observed on or about October 15, 2015, where at least 20 employees were exposed to trip and fall hazards in the Press #4 area. Where employees walk and work, an electric oil pump was in an open aisle way in the path of employee travel.      b) Press #5 Area: This violation was observed on or about October 15, 2015, where at least 20 employees were exposed to trip and fall hazards in the Press #5 area where an electric oil extraction pump was placed in the pathway to the entrance of the press.

29 CFR 1910.23(a)(2):  Every ladder way floor opening or platform was not guarded by a standard railing with standard toe-board on all exposed sides with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening:      a) Extruder Press #5: This violation was discovered on October 15, 2015, where at least 20 employees were exposed to falling from an elevated platform into a pit due to the ladder opening not being guarded.

The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6:    Section 3.4.4, Compressed Gas Association Pamphlet P-1-1965, as adopted by 29 CFR 1910.101(b): Compressed gas cylinder(s) not in use were not properly supported to prevent them from being knocked over:      a) Extruder Press #3: This violation was observed on or about October 15, 2015, where acetylene cylinders were stored upright but were not properly supported to prevent them from being knocked over, exposing employees to fire and projectile hazards.     b) Extruder Press #5: This violation was observed on or about October 15, 2015, where acetylene cylinders were stored upright but were not properly supported to prevent them from being knocked over, exposing employees to fire and projectile hazards.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.     a) Maintenance Department: This violation was observed on October 15, 2015, where maintenance employees were exposed to a fall hazard of four or more feet to ground level below due to a missing guardrail while entering the elevated storage area in the maintenance shop area.       THE EMPLOYER WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1910.23(c)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316092501, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 06/07/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 1715 SANDY LAKE ROAD, CARROLLTON, TEXAS 75006.
