29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:   On or about 10/14/2015, and at times there prior to, supervisors and employees were not provided NFPA 70E training that included required arc flash PPE and clothing and the use of insulated tools specific to various scenarios of work as described in NFPA 70 E.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided and/or using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:   On or about 10/14/2015, and at times there prior to, employees performed voltage testing on energized equipment rated 120 to 480 V. Employees were not using rubber insulating gloves in combination with leather gloves, arc flash rated face shield or hood, and ear canal caps.
