29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for de-energizing circuits and equipment shall be determined before circuits or equipment are de-energized. On or about February 2, 2016, and at times there prior to, at MCLB Barstow Pump House Building 325, the employer did not ensure there were adequate safe procedures in place to de-energize circuits and equipment. a) An employee was exposed to foreign voltage (120V) not identified in the established de-energizing procedures after securing the main power source.

29 CFR 1910.335(a)(1)(ii): Protective equipment was not maintained in a safe, reliable condition and/or was not periodically inspected or tested: On or about February 2, 2016, and at times there prior to, the employer did not ensure that arc-rated gloves requiring periodic inspections, were turned in and tested to ensure the limitations and the rating of the gloves were not compromised.
