29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Cell 5 Area, 1 Tuscarawas Road, Beaver, Pennsylvania, 15009: On or about October 8, 2015, the chute guard on a Compu-Cut flat cable, wire, & tubing cutter (model 42B, serial number 4200173) was not equipped with roller guards. The chute guard was missing three roller guards which made it possible for operators of the cutting machine to access the blade area. Employees operating the machine were exposed to cut-by hazards.
