29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation: On or about October 23, 2015 at Marine Builders, it was noted that the logs did not provide adequate descriptions of the injuries, failing to explain the injury, the location of the injury and what caused the injuries. Additionally, the second injury, on what is presumed to be the 2015 log failed to provided information to classify the injury.
