Scaffold did not have base plates or mudsills next to the utility excavation on or about 10/8/2015.  (c) Worksite was located at 212 Third Street, Jersey City, NJ 07032. Scaffold was erected using frames that had been damaged / cut / broken on the stairs on or about 10/8/2015.  (d) Worksite was located at 212 Third Street, Jersey City, NJ 07032. Scaffold was erected using planking that was cracked / rotted on the underside of the planking near the basement ramp  on or about 10/8/2015.  Violations were made on or about 10/8/2015.
