29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. (a) Back and front side of the house - On or about May 22, 2024, the employer did not ensure the worksite was clean and clear due to work material scattered throughout the site, exposing employees to trip and fall hazards. (b) Back and front side of the house - On or about May 22, 2024, the employer did not ensure the worksite was clean and clear due to protruding nails from wood boards throughout the site, exposing employees to impalement hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems. (a) West and South side of roof - On or about May 22, 2024, the employer did not ensure that the employees working from the roof more than 12 feet high without a fall protection system, exposing them to fall hazards.
