29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   On or about at 10/19/15 located at 4160 Carramore Dr., Cumming, GA: Employees were installing roofing shingles on the 6/12 pitch roof of a house that was under construction while working at the edge, the employer did not ensure that the lanyards used for fall protection were attached to the back and centered at the shoulder. Employees were exposed to fall hazards of approximately 35 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about at 10/19/15 located at 4160 Carramore Dr., Cumming, GA: The employer did not retrain his employee that was exposed to fall hazards of up to 35 feet and a 6 / 12 pitch while installing shingles on to the roof. Employees were using body belts and connected to the front of the waist.
