29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: At the time of inspection, the employer failed to ensure that the 7 Deuce unloading system's loads were secured before raising the loads more than a few inches. The top hooks were about 1.75" high, and the bottom hooks were only about 1.25" in height.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the time of inspection, the employer failed to provide guarding to machinery including, but not limited to the following machines in the Finishing Department: a) The 7 Deuce shaft extractor did not have a bump stop, b) The 7 Deuce shaft extractor did not have a guard in place between the unload panel and the shaft extractor. There was a 1'' gap between the shaft extractor and the unload panel when the shaft extractor was raised, creating a pinch point.
