29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     At the new residential construction site, employees using pneumatic nail guns were not protected from eye injuries by wearing protective eyewear.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.    At the new residential construction site, employees were installing shingles on the roof without fall protection exposing them to an unprotected fall hazard of more than 10 feet to the lower level.

29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At the new residential construction site, employees used ladders that were not extend at least 3 feet above the upper landing to access the working area exposing them to a fall hazard up to approximately 10 feet to the ground below.
