29 CFR  1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On the north side of the construction site, employees engaged in roofing activities use pneumatic nail guns without wearing eye protection were expose to struck-by hazards and eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).     On the north side of the construction site, employees engaged in roofing work without attaching their harnesses to the lifelines were expose to unprotected fall hazards of approximately 9 feet to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On the construction site, employees use extension ladders to access and egress the roof and the side rails did not extend three feet over the roof landing surface.  The employees were expose to potential fall hazards greater than six feet.  1) On the west side of the construction site extension ladder not extended 3 feet above the landing,  2)  On the east side of the construction site extension ladder not extended 3 feet above the landing,  3)  On the north side of the construction site extension ladder not extended 3 feet above the landing.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.    On the  construction site, the employer did not develop or provide fall protection training so employees could recognize and eliminate fall hazards while engage in roofing work.
