29 CFR 1910.147(c)(1): The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperable. a) Cutting Area: The employer did not develop, document and utilized an energy control procedure (lockout/tag out program), conduct periodic audits and did not provide training on the energy control procedures to ensure that all required elements of the procedure were understood by employees and that the knowledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees performing servicing(changing saw blade) and repair work on a machine or equipment, such as but not limited to 4 Smart Products horizontal band saws, on or about 10/19/15. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
