29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about October 14, 2015 for the employee who sustained injuries while performing work activities on top of the cutting table with the Messer, model TITAN, Serial No. TITAN14/4p-08-5172, Plasma cutting machine in auto cut mode at 1617 317th Street, Vermillion, South Dakota. (b) On or about October 20, 2015 and at times prior to that date, for the employees exposed to caught-in and/or struck-by hazards while performing work activities inside the master carriage track and cutting table with the Messer, model TITAN, Serial No. TITAN14/4p-08-5172, Plasma cutting machine in auto cut mode at 1617 317th Street, Vermillion, South Dakota.
