29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jobsite, 400 Park Avenue, Bldg. 12, Natrona Heights, PA: On or about October 21, 2015 the portable ladder used to access the roof area did not extend at least 3 feet above the upper surface. This condition enhanced the exposure to fall hazards of up to 18 feet when accessing/egressing the ladder.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training: (a) Jobsite, 400 Park Avenue, Natrona Heights, PA: On or about October 21, 2015 the employer had not provided retraining to employees who were using extension ladders. The employees had the ladders set up incorrectly and were using them to access the upper roof area indicating they had not maintained the understanding and knowledge acquired by prior training.
