29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement:    At the front of this commercial construction site, at the center scaffold section, where bricklaying operations were conducted, scaffolding was constructed with one of the base plates on a wooden block utilized for leveling. The scaffold footplate was partially off the wooden base support and subject to displacement.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toe boards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    At the front of this commercial construction site, at the center scaffold section, where bricklaying operations were conducted, employees were exposed to falling objects as bricks were stacked on the scaffold planks and not positioned as to prevent them from falling onto another employee below.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:     At the front of this commercial construction site, at the center scaffold section, where bricklaying operations were conducted, employees were exposed to falling objects as bricks were stacked on the scaffold planks and toe boards were not provided as to prevent them from falling onto another employee below.

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:    At the front of this commercial construction site, on a scaffold section where bricklaying operations were conducted, a brick cutting saw was utilized with an improperly repaired power cord.  The power cord consisted of two different types of electrical cord. Employees were exposed to electrocution.
