29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm): On or about March 16, 2016, at the rear of this renovation site an employee worked from an incomplete scaffold platform. The employee was exposed to a fall through the scaffold greater than 10 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. On or about March 16, 2016, at the rear of a building under renovation, a complete guardrail system was not installed prior to employees working from platforms. Cross braces were used as a top rail; mid-rails were not installed to prevent employees from falling more than 10 feet.
