29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 10/21/2015 at the 2520 Main Street Construction Project, employees were installing trusses with no fall protection system in use.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 10/21/2015 at the 2520 Main Street Construction Project, employees were using extension ladders to access and descend to and from the upper level and the ladders did not extend 3 feet past the upper landing surface. Fisher Builders, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314406612, citation number 01, item number 001 and was affirmed as a final order on 9/13/2011, with respect to a workplace located at 119 Hebron Avenue in Glastonbury, CT.
