29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that; On or about October 21, 2015 the employer failed to ensure that employees performing work inside an excavation were protected from cave-ins. Employees were grading substrate material in an excavation measuring 17 feet in width and 11 feet in height. Trench boxes or shoring was not present. Sloping and benching of the earth was also not performed. This with respect to a worksite located in the 9580-9600 block of Kickapoo Road in Streetsboro, OH.
