A) Burton Scot Contractors on or about March 21, 2018, at the site located along State Route 2 between mile marker 165 and 166 in Elyria, Ohio: The employer did not ensure an employee's personal fall arrest system was secured to a JLG 860SJ Aerial Lift (Sun Belt Rental). An employee operating from the lift, so as to reach a ledge to perform tree trimming activities, was exposed to a fall of up to 86 feet to the ground below.
