State Paper & Metal Co., Inc. located in Toledo, Ohio: On or about October 20, 2015, the employer did not ensure that the Mustang skid steer loader #2, used by employees to move paper products in the baler area, was equipped with a functioning back up alarm, exposing employees to struck-by and crushing hazards.       Among other acceptable methods, a feasible abatement method would be to follow the guidelines stated in the Association of Equipment Manufacturers (AEM) manual SL02-12 Skid Steer Safety Manual for Operating and Maintenance Personnel. Repair the existing back up alarm so it is functioning.
