Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: a) State Paper & Metal Co., Inc. - Toledo, Ohio: On or about June 21, 2016, the guards over the chains on the mast of the Hyster forklift (Model: S50XL; SN: C187V05926P) were rotated 180 degrees, which created a larger gap between the chain and the guard below the pulley, exposing employees to an ingoing nip point. b) State Paper & Metal Co., Inc. - Toledo, Ohio: On or about June 21, 2016, the name plate on the Hyster forklift (Model: S50XL; SN: C187V05926P) was not updated to show that there was no sideshift on the forklift. c) State Paper & Metal Co., Inc. - Toledo, Ohio: On or about June 21, 2016, the sideshift lever on the Hyster forklift (Model: S50XL; SN: C187V05926P) was cut off.
