OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to crushed-by and struck-by hazards:     a) Berry Plastics, 918 8th Avenue, Columbus, Ga:  Employees who operated a variety of hoists including but not limited to: Hoist 1; Hoist 2; Hoist 3; Hoist 7; Hoist 8; Hoist 10; Hoist 11; Hoist 12; Hoist 13; Hoist 16; Hoist 17; Hoist 19; Hoist 20; Hoist 21; Hoist 24; Hoist 26; Hoist 32; Hoist 37; Hoist 39; Hoist 40; Hoist E3M; Hoist TLE; Hoist T2LLL; Hoist T2UU; Hoist E3E; Hoist RC; and Hoist PPR were exposed to crush by and struck by hazards.  Employees were operating hoist and multiple hazards were observed, such as but not limited to: hoists were modified and were not checked thoroughly for the new rated load by a qualified engineer or the equipment manufacturer, the rated load were not plainly marked on each side of the hoists, trolley strops were not provided at the limits of travel, power control or mechanically controlled braking means were not capable of maintaining safe lowering speeds of rated loads, pendant control boxes were not constructed to prevent electrical shock, unsafe conditions identified during the frequent or periodic inspections were not corrected before operation of the hoists were resumed, and repairs or replacements were not provided promptly as needed for safe operation of hoists.
