29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) On or about October 15, 2015, the work area was disorganized and unkept, exposing employees to punctures from protruding nails and slips, trips and fall hazards.

29 CFR 1926.20(b)(2): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about October 15, 2015, and at times prior to that date, employees were exposed to injuries from falls, while conducting residential construction activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface. Jino Carrera Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1056876, Citation Number 1, Item Number 1, and was affirmed as a final order on May 29, 2015, with respect to a workplace located at 2312 Washo Circle, Colorado Springs, CO 80909.
