29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:   a)    At the 318 Mall Blvd. Building 1 jobsite: On or about October 22, 2015, and times prior thereto, the employer exposed its employees to fall hazards of 13 feet 4 in. to the ground, in that a masonry worker on the second buck of the north supported scaffold was engaged in bricklaying activities  without two top-rails.     b)    At the 318 Mall Blvd. Building 1 jobsite: On or about October 22, 2015, and times prior thereto, the employer exposed its employees to fall hazards of 13 feet 4 in. to the ground, in that a masonry worker on the second buck of the south supported scaffold was engaged in bricklaying activities  without a mid-rail and two top rails.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   a)    At the 318 Mall Blvd. Building 1 jobsite: On or about October 22, 2015, and times prior thereto, the employer exposed its employees to fall hazards of 13 feet 4 in. to the ground, in that employees performing masonry work on two supported scaffolds were not trained by a qualified person on recognizing related hazards, and the procedures to control and minimize such hazards.
