29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:    On or about October21, 2015, at the job site an employee was observed working on a mobile scaffold which the casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold.  The employee was exposed to fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about October 21, 2015, at the job site employees were observed working at the edge of a building approximately 34 feet above the ground.  The employees were not wearing personal fall arrest and had no other type of fall protection systems provided.  The employees were exposed to fall hazards.
