29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). The employer used an extension ladder that was not properly placed so that the angle was further than required for safe use.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. The New Design Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 892259, citation number 1, item number 1 and was affirmed as a final order on April 3, 2013, with respect to a workplace located at 2190 Higdon Ferry Road, Hot Springs, Arkansas 71913. The employer did not ensure employees using pneumatic nail guns were protected by the use of protective eyewear, exposing them to the hazards of flying projectiles.

Employees were observed working on a residential roof with a pitch of 8:12 at heights greater 6 feet without the use of fall protection, exposing employees to fall hazards.
