29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to items (d)(3)(i) through (d)(3)(vi) of this paragraph.   The employer is failing to protect employees from engulfment and atmospheric hazards when entering permit required spaces.  This was most recently documented where employees entered #6 formation pump sump to perform job duties such as calibrating the pH probe, repairs and cleaning.  Specific procedures had not been developed and implemented for entry.

This was most recently documented on and before 11/19/2015 where employees entered a permit required confined space in the # 6 Formation Pump Sump that contained engulfment and atmospheric hazards to calibrate the pH probe. The employer did not ensure that entry was conducted with an attendant.

This was most recently documented on and before 11/19/2015 where an employee entered a permit required confined space in the #6 Formation Pump Sump that contained engulfment and atmospheric hazards to calibrate the pH probe. The employer did not ensure that an entry permit was completed prior to entry into the space.
