In the production area of the plant an employee was performing maintenance activities on the Single Fold # 2 machine which required lockout and the employee had not; 1. Prepared for shutdown knowing the magnitude of the energy, hazards of the energy to be controlled, and the methods to control those hazards. 3. Isolated all energies by utilizing energy isolating devices on the machine. 5. Verifying that the energy control devices have isolated and deenergized the correct machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 10/26/2015, Guards on the #1, 1/4 folder were not inter locked or secured to prevent operators from being inside of guard door while machine was operating. Machine guarding gap on the side of the electrical panel permitted access to the sharpening stones during operation. b) On or about 10/26/2015, No guards were protecting the slitter blades on the core machine. Pinch (nip) points were also exposed on the equipment. c) On or about 10/26/2015, Folder #2 had an unguarded nip point by the compression head. The blue end guard had a safety interlock switch that was defeated. No guarding was provide for the side of the machine to keep people out from under the machine. The emboss rolls did not have a nip guard. d) On or about 10/26/2015, #2 Quarter fold was being run with a defeated safety interlock switch on the 2nd guard door. The draw rolls and cut off rolls on the top of the former were not guarded.
