29 CFR 1926.102(a)(1):   Eye and face protective equipment was not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    North side of apartment complex construction.  On or about October 27, 2015, an employee was exposed to the hazard of being struck by flying projectiles, while using a pneumatic nail gun to install wood cornices without the use of eye protection.

29 CFR 1926.453(b)(2)(v):  A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).    North side of residential construction.  On or about 27 October, 2015, an employee was exposed to the hazard of a fall of over 6 feet to the ground level below, while he was attaching wooden cornices to the roof, from the basket of a Genie S65 telescopic boom aerial platform, without the use of a body belt or personal fall arrest system.
